Axess Family Services

Overview

Axess Family Services provides integrated healthcare and human services designed to support the whole person and strengthen families and communities. Our services include primary medical care with on-site lab services, dental, vision, pharmacy, and behavioral health care, along with substance use treatment, medication-assisted treatment, and residential SUD services. We also offer counseling, case management, community health worker support, housing assistance, domestic violence shelter services, and emergency shelter programs. Additional supports include veteran services and programs that address food insecurity, transportation, and access to public benefits. Located across Northeast Ohio and parts of Pennsylvania and Michigan, Axess Family Services uses a person-centered, community-focused approach to remove barriers to care and create pathways to long-term health, stability, and well-being.
